Charting mistake of the day
A large part of my job is reading everyone else's charting and summarize it for the rest of the staff in the morning meeting. We are all overworked and most of us don't love having to write on every patient. So it shouldn't be a surprise that I read some pretty interesting sentences. This was my favorite of the day.
"37 year old man presenting for med management...he has a twin aged 14 years old and they both live with his ex-girlfriend"
I think he meant "twins" instead of "twin"
